The fallopian tubes are the place for meeting of sperm and egg. If any pathology is there it will impact your chances of getting pregnant. Tubal disease contributes to 25-35 % of female infertility. The main cause for tubal infertility is inflammation of tubes. Pelvic inflammatory disease is the most common cause for tubal disease, mostly caused by chlamydia trachomatis infection.

If one or both tubes are blocked they will impact your fertility greatly. Blockage of tube can be proximal and distal. Proximal blockage could be because of PID or endometriosis. Distal blockage most commonly due to hydrosalphinx.

What is Hydrosalpinx?

A hydrosalpinx is a particular type of tubal blockage in which the tube is obstructed near its fimbrial end – the
open, outside end of the tube. With hydrosalpinx, the tube becomes filled with clear watery fluid.

Hydrosalpinx cause tubal blockage and the dirty fluid of hydrosalpinx can be toxic to embryo and can wash off embryo. The best method to detect hydrosalpinx is by routine 2D transvaginal scan.

The ideal management of hydrosalpinx is IVF and embryo transfer after tubal occlusion.

How are Tubal Diseases treated?

HSG (hysterosalpingography ) can help in screening of tubal blockage to women with no suspicion of co-morbidities like PID, endometriosis, hydrosalpinx.

In women with suspected comorbidities should advise laparoscopy with dye test so that any treatable tubal or pelvic pathology can be evaluated and managed at the same time. Evaluation of pelvic infection ( Chlamydia / TB ) also should be done.

Proximal tubal occlusion may get treated by hysteroscopic tubal cannulation. In case of distal tubal disease treatment depend on stage of tubal disease and involvement of endosalpinx & tubal wall. The ideal management of hydrosalpinx is IVF and embryo transfer after tubal occlusion.
